Ahead of your upcoming color analysis consultation, please complete the information below and return it prior to your appointment. This allows me to prepare and ensure the most accurate and personalized results possible.

Appointment Preparation
To ensure the most accurate results during your consultation, please follow the guidelines below:
- Please arrive with no makeup. If you are coming from another commitment, feel free to bring makeup remover and plan to arrive a few minutes early to remove it before your session begins.
- If you dye your hair, it is helpful (when possible) to have a bit of visible natural root growth for reference.
- Please wear a white top if available, as this helps with color assessment and keeps the focus on your natural features.
- If possible, please avoid wearing earrings, necklaces, or other facial/neck accessories during your appointment, as these can affect color reflection and assessment. These small details make a meaningful difference in the accuracy of your results.
Required Photos:
Please attach 5–10 photos of yourself from childhood and/or early teens that clearly show your natural hair color. These images are an essential part of the analysis process and help ensure accuracy.
Photo guidelines:
- Natural lighting preferred
- Minimal or no makeup (when possible)
- Clear view of your face
- A variety of ages/stages is helpful
Consultation Goals
- What are you hoping to achieve through color analysis?
- What challenges do you currently face with color choices?
(For example: clothing feels unflattering, makeup doesn’t look quite right, uncertainty about what suits you, wanting to feel more polished or confident, etc.)
Please send this information along with your photos at least 5 days before your appointment.

Client Agreement & Consultation Consent
By signing below, you acknowledge and agree to the following:
1. Scope & Nature of Services
This consultation includes a personalized color analysis based on visual assessment, client-provided information, and reference materials (including photos). The goal of the service is to identify a color season that best harmonizes with your natural features and highlights the colors that are most naturally flattering to you.
Color analysis is a personalized and interpretive process. It is not a medical, dermatological, or scientifically diagnostic service. All results are stylistic and based on professional judgment.

2. Client & Service Provider Responsibilities
Service Provider Responsibilities:
• Provide a thoughtful and careful color analysis based on training and professional judgment
• Use client materials (photos, information, and in-person assessment) to support accuracy
• Avoid rushing or guessing results in situations where further evaluation is needed
• Respect and carefully manage all client materials, using them only in accordance with the permissions outlined in this agreement
Client Responsibilities:
• Provide accurate and complete information to the best of your ability
• Submit required photos prior to or during the consultation process
• Attend the consultation prepared as instructed
• Understand that results are carefully determined, and in rare cases may be reviewed further to ensure the highest level of accuracy

3. Accuracy & Results
While I strive for the highest level of accuracy, color analysis is not an exact science, and 100% certainty cannot be guaranteed. In rare cases, a final determination may not be reached during your session. If that occurs, I will continue to assess your results within my organization to ensure accuracy rather than provide a rushed or uncertain conclusion.

4. Commitment to Quality Over Speed
My priority is to provide thoughtful and accurate results. I will not guess or rush the process simply to deliver same-day results if further evaluation is needed.

5. Client Understanding
You acknowledge that, while unlikely, it is possible to leave the consultation without a final result on the same day. In these rare cases, your final results will still be provided once a conclusion has been reached following further analysis.

6. Photo & Video Consent
I may take photos and/or videos during your consultation for documentation, training, and internal review purposes.
Please indicate your preference regarding public use of this content:

☐ I do NOT consent to my images/videos being used publicly. I understand they may still be used internally for assessment and record-keeping.

7. Required Client Photos
As part of the color analysis process, you are required to provide recent and past photos of yourself, including images that reflect your natural hair color over time. These materials are necessary to support an accurate and thorough assessment.
All client-provided images will be used solely for consultation, reference, and internal analysis purposes. They will not be shared, published, or used for marketing without your explicit additional consent and will be stored securely.

8. Release of Liability
By participating in this consultation, you acknowledge the interpretive nature of the service and release the provider from liability related to outcomes or dissatisfaction with results.

9. Recording & Content Use Restrictions
The Service Provider may record portions of the consultation for documentation, educational, quality control, and business purposes. Clients may receive selected clips or excerpts at the sole discretion of the Service Provider.
Clients are not permitted to independently audio record, video record, screen record, or otherwise capture the consultation without prior written consent from the Service Provider.
Any clips, recordings, screenshots, or materials shared with the Client by the Service Provider are for personal viewing only unless otherwise agreed in writing. Clients may not:
• Post, publish, upload, distribute, or share the full consultation recording in any form
• Share or publish any audio from the consultation, whether in full or in part
• Alter, reproduce, sell, or commercially use any provided content
• Remove context from clips in a way that misrepresents the Service Provider or the consultation
Clients may share short visual clips, screenshots, or excerpts provided that no consultation audio is included.
All recordings, materials, methods, and consultation content remain the intellectual property of the Service Provider.
